Dear Friends and Supporters:
The 56th Annual Physicians Ball of the Polish-American
Medical Society will be held on February 11, 2006, in
the Gold Coast Ballroom of the Drake Hotel in Chicago.
Once again we will be asking you to contribute to the
success of this event.
You can help make this event successful by:
· Attending the Ball with your spouse/significant other,
family, colleagues, and business associates.
· Placing a personal greeting and/or advertisements in
our program book.
· Sponsoring any of the ball expenses (wine, band, sweet
table, etc.)
· Donating items for the silent auction.
Our Society is a professional organization of Polish-American
physicians in the Chicago area and has been in existence
for 60 years.
The proceeds from the Physicians 2006 Ball will go towards
the Academic Fund of the Polish-American Medical Society
and the Abakanowicz Fund.
Our tax-exempt Federal Identification Number is 366-112-686.
Enclosed please find an advertised form as well as a silent
auction form.
All advertisements, gifts, and payments are due by January
25, 2006.
