Fund of the Polish - American Medical Society (PAMS) in Chicago
Statute
Goals of the Fund:
The purpose of the Fund is to provide
financial help towards tuition costs of the course preparing
medical graduates from Poland to pass American medical licensing
examination. This Examination Fund was established by the
board of PAMS in Chicago on October 4, 2006. The initial Fund
amount is $ 10,000 from the proceeds of the 56th Physician’s
Ball held on Feb 11, 2006. Subsequently, funds will be accumulated
through 10 percent of the proceeds from future Physicians’
Balls, private contributions, business donations and other
means, e.g. fund drives. All members of PAMS may solicit such
donations.
Rules of the Fund:
Graduates of medical schools in Poland
who live in the Chicago area and are preparing to take the
medical licensing examination may apply for a stipend as financial
help towards tuition costs of the appropriate course, e.g.
Kaplan’s. The amount of these stipends will be decided twice
each year, based on funds available, number of applicants,
their financial need and the tuition cost. The PAMS board
will approve these distributions by majority vote.
Applicants responsibilities
are:
• To submit a completed application
form (available at www.zlpchicago.org) specifying medical
school, work experience and financial need
• To include one copy of their medical diploma
• To provide confirmation of their legal immigration status
in the USA
• To secure two recommendation letters
• To enclose two photographs
• To provide name and address of the course to be taken, tuition
amount, registration and payment receipt (if applicable) or
the name and address of the organization to which the PAMS
tuition check should be payable.
The deadline for submitting applications will be Jan 15 and
July 15 each year.
