POLISH-AMERICAN MEDICAL SOCIETY IN CHICAGO ACADEMIC FUND
I - SCHOLARSHIP PROGRAM DESCRIPTION
The Polish-American Medical Society
Academic Fund provides tuition scholarship to qualified full
time graduate students from Chicago for medical, dental or
veterinary studies in the United States and in Poland.Scholarships
range from $ 2,000 to $ 7,000.
II - ELIGIBILITY
Applicant must:
- be a United States citizen or a permanent resident of Polish
descent from Chicago metropolitan area
- be (or be admitted) as a medical, dental or veterinary school
full time student in the United States or in Poland
- have a minimum GPA of 3.0
III - SELECTION CRITERIA
Selection is based on:
- academic excellence
- applicant's other achievements, interests and motivation
- applicant's interest and involvement in the Polish-American
community
- financial need.
IV - APPLICATION PROCEDURE
Tuition scholarship application forms
are available from October 1 through December 30
1. Submit application, $ 25 non-refundable application fee
and supporting materials to the Polish-American Medical Society
by mail. (Click

2. Submit the following supporting materials:
a) A personal statement. Discuss your
background, interest, academic and career goals, community
involvement.
b) Two passport photos 1.5 x 1.5 inch.
Print your full name on the reverse side of each.
c) Undergraduates: official transcripts
of all undergraduate work through summer preceding application.
d) Graduates: submit copies of all degree
diplomas received + transcripts connected with most recent
degree through summer proceeding application.
e) Two letters of academic reference
submitted on letterhead.
f) Proof of ancestry. Use birth certificates,
passports, naturalization papers or other official document
to show the descent from the ancestor who was born in Poland.
If documentation is not available, a letter from a prominent
member of the community verifying your Polish ancestry will
be accepted. (Letters from relatives should not be used)
g) Financial information form.
h) Other supporting materials e.g.,
awards, publications.
- For information of receipt of your
materials include a self-addressed stamped post-card.
- A student can receive funding through
the Academic Fund no more than twice. Only one member per
immediate family may receive a tuition scholarship during
a given academic year.
V - DEADLINE
Application, fee, and all supporting
materials are due by January 15 each year.Scholarship notifications
are announced in writing on or about May 15 each year.
