The 53rd Annual Physicians’ Ball of the Polish-American Medical Society will be held on Saturday, February 15, 2003, at the Grand Ballroom of the Palmer House Hilton Hotel in Chicago. We are asking your company to contribute to the success of this event by providing us with an unrestricted educational grant.

Our Society is a professional organization of Polish-American physicians in the Chicago area, and has been in existence for 56 years. The tax exempt Federal Identification Number is 366112686.

The proceeds from the Physicians’ Ball 2003 will go towards the completion of our Society’s Academic Scholarship Fund for Chicago area medical students. The co-recipient of our profits will be The Lira Ensemble.

The Chicago Polish Community is the second largest in the world. Our doctors provide services to thousands of Polish patients. We hope you will help support our community of doctors and patients.
